model number on older mh

I need to find the model number on my 1978 New Horizon Mobile home.The Serial number still appears on the page beside the breaker box in MAster bedroom but the top portion is gone. That must have been where it was..located. We received damage in the hurricane and I need this for Fema/sba. Thanks for any help you can give me .
